Incident Report
Also: safety report, near-miss report
An incident report is the documented record of any safety, security, or medical event during a production: an injury, a near-miss, a security intervention, a medical response, an environmental release. Incident reports capture the time, location, witnesses, photos, and resolution. OSHA-recordable incidents flow into the annual 300 log. Anonymous reporting channels, where the reporter chooses what they disclose, are increasingly standard for safeguarding and harassment reporting.
